During morning observations, the client followed directions on the first prompt while participating in his morning math stations. During midday observations, the client communicated with his teacher in an appropriate manner while outside for recess. During afternoon observations, the client exhibited difficulty following directions on the first prompt by not staying in his designated bus are The client appeared to run around utilizing unsafe measures. QMHP-C greeted the client upon arrival to the school setting. During morning observations, QMHP-C provided the client with positive reinforcement in an effort to continue making wise choices. QMHP-C encouraged the client to continue following directions on the first prompt. During midday observations, QMHP-C provided the client with verbal praise for utilizing positive communication skills. QMHP-C encouraged the client to continue communicating with his teacher in an appropriate manner. During afternoon observations, QMHP-C redirected the client to his seat. QMHP-C reiterated the importance of following directions on the first prompt. QMHP-C engaged the client in discussion surrounding the importance of staying in his designated area. The client responded well towards TDT services as evidenced by participation and engagement. During morning observations, the client agreed to continue to follow directions on the first prompt while completing his class work. During midday observations, the client continued communicating with his teacher in an appropriate manner while at recess. The client agreed to continue to track his behavior on his self-monitoring chart. During afternoon observations, the client was unreceptive to redirections by running through the hallway. The client was displayed the ability to calm down during redirection. The client agreed to follow directions on the first prompt. The client returned to his designated bus area. QMHP-C will continue to redirect and prompt the client to control his impulse as evidenced by staying in his seat. QMHP-C will continue to encourage the client to communicate/respond to authority figures in an appropriate manner by verbalizing his thoughts and feelings daily. QMHP-C will continue to encourage the client to follow directives from authority figures within 3 prompts. QMHP-C will continue to provide support to the client within the school setting in an effort to maintain in school learning.
